Re: Termination of PSC/CUNY Welfare Fund Term Life Insurance-Reminder
The University Benefits Office has asked us to alert you to the attached
information about life insurance. Employees who currently carry life
insurance were sent information about the termination of PSC/CUNY Welfare
Fund Life Insurance, and the successor carrier, NYSUT along with an
enrollment form, payroll deduction form and envelope.
Please note that the deadline to enroll in NYSUT is October 15, 2003.

PSC-CUNY Welfare Fund Members Only! URGENT
Effective November 1, 2003, the PSC-CUNY Welfare Fund Basic Life Insurance
and Optional Life Insurance Program will be discontinued.
Recently, as part of the comprehensive restructuring of Welfare Fund
benefits, the Welfare Fund Board of Trustees voted to implement a number of
significant changes. One of these changes was the discontinuance of the
PSC-CUNY Welfare Fund Basic Life Insurance and Optional Life Insurance
Program.
As a one-time offer, current participants have an opportunity to transfer
existing coverage to a premium paying Group Term Life Insurance Plan
offered through the New York State United Teacher (NYSUT) Benefit Trust.
As an added benefit to this one-time only offer, NYSUT has made
arrangements with the underwriter, The United States Life Insurance Company
in the City of New York, to waive all standard underwriting questions.
Payroll deduction is the only payment method available, if you wish to take
advantage of this one-time only offer. Employees on unpaid leave are not
eligible for payroll deductions.
Please note, if you wish to participate in this one-time offer, you must
complete and return the forms included in the enrollment package dated
August 27, 2003, which was mailed, directly to your home address from the
Welfare Fund. It is urgent that the completed forms are returned by
October 15, 2003. In the middle of the insert page in the package you will
find a box which contains the amount of your present life insurance
coverage and the cost of the annual premium for NYSUT Term Life Insurance.
If you wish to purchase additional levels of life insurance, (which are
sold in $5,000 units), and are under age 65 contact NYSUT member benefits
at 1-800-626-8101. Cost for additional levels above what you are presently
covered for vary by age and may require underwriting.
A non-response will be considered as your declination of this offer. You
will not have life insurance coverage unless you complete the enrollment
information and the payroll deduction authorization card and return both by
October 15, 2003, in the business reply envelope addressed to the claims
administrator:

In an effort to insure that you have received a package, a second mailing
will be sent to your home, which will contain all of the information that
you need as well as customer service numbers.
