Re: PSC/CUNY Additional Benefits - Reminder
This is to remind you that the PSC/CUNY offers additional benefits that are
automatically included when you enroll in one of the health plans below.
For GHI/CBP Members
The CIGNA major medical supplement is designed to provide you with limited
protection to enhance your coverage through GHI. This plan covers items
that are medically necessary but are not completely reimbursed by GHI. This
plan has a deductible, and the amount of the deductible depends on whether
or not you purchased the optional rider from the City health insurance.
Check your paystub to determine if you purchased the optional rider. If you
purchased the optional rider then the individual deductible is $500 and the
family deductible is $1000. If you did not purchase the rider, the
individual deductible is $2000 and the family deductible is $4000. After
the deductible is met, CIGNA pays 80% of reasonable and customary charges.
The claim forms for CIGNA may be obtained in the Human Resources Office.
For HIP Prime HMO and POS
Private duty nursing and appliances, such as wheelchairs and crutches, are
covered.
Complete information about these and other benefits is available through
the PSC/CUNY Welfare Fund and can be obtained by calling (212) 354-5230.
Optional Benefits:
The following are two optional benefits programs offered by the PSC/CUNY
that are available through payroll deductions and which you are strongly
encouraged to consider:
Catastrophe Major Medical Plan
A $2 million Catastrophe Major Medical Insurance Plan may be purchased from
Marsh Affinity Group (formerly Wohler’s). Premiums are based on the
employee’s age. The policy provides coverage for the employee’s spouse, and
children who are unmarried, dependent full-time students up until their
27th birthday.
Optional and Basic Long Term Disability Plan
All full time instructional staff are covered for a free basic disability
plan offered by Standard Insurance. The basic income replacement benefit
begins after six months of total disability and continues for five years.
This plan covers 50% of your pre-disability salary up to a maximum of $2500
per month. You are strongly encouraged to purchase optional long-term
disability, which increases the free benefit to 60% of your pre-disability
salary to a maximum of $5000 per month and continues up until age 65. If
you should become disabled, the optional plan also places 10% of your pre-
disability salary into an account set up for you by TIAA-CREF.
Application materials for these two plans can be obtained in the Human
Resources Office.
